Custom Order Form

Custom order surfboards has never been easier than here at catalyst.

All you have to do is fill out the Custom Order Form below, submit the form, and hold tight while we review your submission and confirm that everything is filled out correctly. We will then send you a PayPal Email Invoice via the e-mail you provided for the required non-refundable down deposit of $150. After payment we will submit your order to ...Lost, Quiet Flight, Mark Richards, or Christenson Surfboards. From that point on we will keep you updated on your order's progress, as well as answer any questions you may have!

*Make sure to think long and hard about your Custom Order Form as changes to your order after submission are forbidden!! Please ask any and all quetions you have prior to us submitting your form to ensure satisfaction and accuracy. If you cancel your custom order after submission the $150 deposit will be forfeited. 

*Please follow these links for Custom Lost Logo Placements, Custom ...Lost Logos, and Lost Art choices for your custom. All these items will have specific serial numbers which you are to provide in the Additional Comments so we know exactly what you want!!
